Health Systems Research and Modeling in Changing Environments: Challenges, Innovations, and Future Directions

Abstract

In today’s era of rapid demographic shifts, climate change, technological disruption and evolving health-service demands, health systems are operating in increasingly dynamic environments. To inform policy and practice, health systems research (HSR) and modelling approaches must adapt accordingly. This review examines the evolving landscape of health-systems research and modelling in changing environments, highlighting key methodologies, innovations, and persistent challenges. We explore how modelling techniques including system dynamics, agent-based modelling, network analysis and hybrid approaches have been applied to represent complex health-system behaviour under change. The review identifies major applications such as pandemic response, climate-health resilience and resource-allocation modelling. We discuss methodological issues such as data availability, model validation, adaptation to shifting contexts and stakeholder integration. Special attention is given to low- and middle-income countries where institutional change, weak data systems, and high rates of change pose additional modelling burdens. We then present future directions and emerging trends: integration of artificial intelligence/machine learning with systems modelling; dynamic updating of models in real time; digital twin for health systems; and enhanced participatory modelling with policy-makers. We also propose that health systems research must evolve to embrace adaptive, complex-adaptive system frameworks and embed equity and sustainability in model design. Key challenges remain in standardisation of modelling approaches, capacity building, data governance and translation of model insights into actionable policy. Ultimately, advancing health systems modelling in changing environments offers significant promise to enhance system resilience, optimize resource use, and support the achievement of universal health coverage and health equity goals.
